At your one-on-one consult appointments their will be discussions about:
- the pathophysiology of diabetes,
- the pharmodynamics of diabetes medication,
- discussion of your latest pathology results in relation to diabetes management,
- the effect of nutrition has on your health and assist in creating a individualised plan,
- blood glucose monitoring and your latest results,
- exercise and lifestyle habits,
- being informed of the latest technology available to assist with managing your diabetes.
- Also, due to the increase of technology used in diabetes management discussion can be made of the use of Continuous Glucose Monitors (CGM's) and Insulin pumps.

It is important to remember that you are not judged when you attend On Point Diabetes as we are working together to support yourself in achieving your goals.
Nurse Practitioners have completed a Master's degree at University and are the most senior and independent clinical nurses in the health care system. A Nurse Practitioner is a Registered Nurse with the experience, expertise and authority to diagnose and treat people of all ages with a variety of acute or chronic health conditions within their scope of practice.
Nurse Practitioners have the ability to provide prescriptions to medications, order xray's and diagnostic tests, refer to medical specialists and allied health services as part of a collaborative care.

Medicare Rebate of $50.05
You need to pay in full and we can process your Medicare rebate online. Your Medicare rebate will be deposited back into the account you have nominated with Medicare, usually the same day.
Please note that there is a fee of $30.00 fee for cancellations at less than 24 hours notice or if you do not attend a booked consultation.
